CONCIERGE MEDICINE & COACHING

A multi-disciplinary & tailored approach to care.

For those seeking real, sustainable transformation in their lives.

Dr. Jay’s approach

A CLEAR, WHOLESOME APPROACH

I bring the perspective of a psychiatrist, healthcare executive, and executive coach to offer clear, whole-person care that is both clinical and practical. I intentionally keep my caseload small, so each relationship gets depth, continuity, and consistent follow-through.

This is not quick, check in care. Your scheduled sessions can be used for psychiatry, coaching, or a blend, based on what will create the strongest next step.

  • Care is personalized, collaborative, and grounded in both clinical expertise and real-world application.

  • A comprehensive, whole-person assessment to understand context, patterns, and goals.

  • An individualized plan that may include medical support, coaching, lifestyle considerations, and integrative approaches—aligned with what works best for you.

  • Consistent guidance and accountability to support healthy habits, clarity, and long-term progress.

Preferred Care Access

I have a small-panel practice designed for people who value consistent, relationship-based care. Patients select one of two service levels:

One 50-minute session per month or two 50-minute sessions per month.

Sessions can be used for psychiatry, coaching, or a mix. Billing runs each month automatically via the patient portal.

This is a payment arrangement for scheduled services, not insurance, and not a medical, or healthcare plan.

Included beyond your scheduled sessions

  • Secure portal messaging for non-urgent questions (SimplePractice)

  • Brief check-ins when appropriate, if something small needs quick support, outside out scheduled time.

  • Coordination with other caregivers when helpful, and with your consent

  • Lab requisitions when clinically appropriate, plus lifestyle and supplement recommendations when appropriate.

  • Administrative letters, such as travel and ESA letter,s when clinically appropriate (not deducted from visit time)

This practice does not provide emergency services. In crisis, call 911 or call or text 988.
